Insurance and Payment Policies

Crosspark Medical, PLLC currently participates with the following insurance companies. Please click the insurance company which you belong and register your name

Policy regarding non-participating insurance plans:

In the event that we do not participate with your particular insurance plan you may still be seen by one of our doctors. Payment in full is expected at the time of service.

Payment policies:

We gladly accept personal checks, cash, Master Card, Visa, and other credit cards and debit cards.

We will file your insurance carrier for services provided to you in our office. You are required to present your insurance card at the time of the visit. Without a current insurance card, we will not be able to file your claims appropriately and you will be responsible for the payment of all charges if your insurance coverage changes. Notify us at the time of your visit. We suggest that you carry your insurance card with you at all times that you come to the office.

Your insurance company requires our office to collect co-payments, co-insurance and deductibles. We request this at the time the service is rendered.

Self-pay patient are seen in our office. Payment for services are required at the time of the visit.

Our office performs many tests, services and procedures in our office if you are concerned that your insurance may not cover certain services, please contact them prior to the date of service. Some services are not covered. You may be responsible for payments.

Laboratory tests are normally
drawn here in our office some. Exceptions are those made by some insurance companies that require you to go to a particular laboratory.
